    Patient-Centered Beliefs Among Patients and Providers in Kazakhstan

    BACKGROUND: Health services in Kazakhstan have undergone radical reforms since its independence in 1991, but levels of dissatisfaction among patients remain high. We investigated whether healthcare providers and patients at one hospital in Astana, Kazakhstan, believe the doctor-patient interaction should be doctor-centered vs patient-centered. METHODS: We conducted a cross-sectional study of the responses to various surveys of 200 patients (response rate, 74%) and 201 healthcare providers (response rate, 86%) in a general hospital in Astana, Kazakhstan. The participants completed a questionnaire containing the Patient-Practitioner Orientation Scale (PPOS) and scales assessing life and job satisfaction, job effort-job reward balance, and patient evaluation of communication. Analyses were restricted to those with valid data on the variables age, sex, and PPOS (147 providers and 150 patients). RESULTS: An overwhelming majority of doctors (80%), nurses (87%), and patients (93%) were doctor-oriented. Among patients, higher patient-oriented scores were statistically significantly associated with higher satisfaction with communication with healthcare providers (P=0.038) and with life satisfaction (P=0.009). CONCLUSION: Only a small percentage of doctors, nurses, and patients reported that their interaction should be patient-centered. This congruence between providers and patients does not explain dissatisfaction, but the finding that patient-centered patients were more satisfied suggests an area for improvement

    Model of cybersecurity means financing with the procedure of additional data obtaining by the protection side

    The article describes the model of cybersecurity means financing strategies of the information object with incomplete information about the financial resources of the attacking side. The proposed model is the core of the module of the developed decision support system in the problems of choosing rational investing variants for information protection and cybersecurity of various information objects. The model allows to find financial solutions using the tools of the theory of multistep games with several terminal surfaces. The authors proposed an approach that allows information security management to make a preliminary assessment of strategies for financing the effective cybersecurity systems. The model is distinguished by the assumption that the protection side does not have complete information, both about the financing strategies of the attacking side, and about its financial resources state aimed at overcoming cybersecurity lines of the information object. At the same time, the protection side has the opportunity to obtain additional information by the part of its financial resources. This makes it possible for the protection side to obtain a positive result for itself in the case when it can not be received without this procedure. The solution was found using a mathematical apparatus of a nonlinear multistep quality game with several terminal surfaces with alternate moves. In order to verify the adequacy of the model there was implemented a multivariate computational experiment. The results of this experiment are described in the article. Β© 2005 - ongoing JATIT & LL

    Development of a gel-forming mixture composition for probiotic encapsulation equipment

    The research is devoted to the development of the composition of the gel-forming mixture and the selection of the percentage of components for obtaining intestinal-soluble seamless capsules with probiotics. The purpose of the research is to develop equipment for the manufacture of capsules obtained by the drip method. Capsules must have good structural and mechanical characteristics, the correct shape and resistance to physical influences, in this regard, it is necessary to select the required components and develop the composition of the mixture to obtain the specified structural and mechanical characteristics of capsules. The authors of the article have developed a methodology and a scheme for conducting an experiment that allows to obtain the characteristics of capsules that meet all requirements. A mixture of food gelatin and sodium alginate was chosen as a gel-forming mixture, which showed the best results in the manufacture of capsules. As a result of the experiment, it was found out that the best results were shown by a mixture with a composition of 1% food gelatin and 1% sodium alginate. The composition of the mixture is selected in such a way that, without dissolving in the acidic environment of the stomach, it passes into the intestine, and in the alkaline environment of the intestine it dissolves and releases the probiotics available in the capsules. In the future, seamless capsules with probiotics will be used in the manufacture of functional products that improve the functioning of the immune system. Based on the experimental setup for encapsulating probiotics, industrial equipment will be developed to produce seamless capsules obtained by the drip method

    In this scientific work is aimed to obtain mathematical tools for solving the problem of finding optimal investment strategies in digital cryptocurrencies (hereinafter referred to as the CX) or a CX set from the side of investor/investors were proposed. The solution was found on the basis of the application of the games theory, the theory of fuzzy sets and artificial neural networks (ANN). The developed model, which allows one to obtain an algorithm for the success forecast assessment of the investment procedure in the CX by the investor, which can be then implemented in one of the modules of the intellectual information system for the CX rates forecasting. The scientific novelty of the results is that for the first time to solve the problem of the CX market evaluation in the context of the CX investment problem, gaming approaches based on solving a bilinear quality game in fuzzy production, as well as ANN were used

    This article shows an installation for encapsulating food products. Ensuring food security, the constant satisfaction of the population's needs for basic foodstuffs, is one of the main directions of ensuring the national security of any state. There is still an open question of obtaining high-quality capsules with properties and characteristics that dissolve in the desired environment, retain the therapeutic effect and at the same time meet the requirements of the consumer (ease of use, efficiency, reasonable cost). An installation for producing microcapsules was made of such parts: they are Mercury ATX Switching Power Supply-450W power supply; aquarium tube; glass washer pump; variable resistor with 10K switch; DC-dc DC voltage stabilizer, LM2596S module; gelatin, sodium alginate solution; electromagnetic mixer; plastic sprayer for spraying water (nozzle); calcium chloride solution. There are many technologies of the encapsulation process. Capsules can be obtained by drip, disk spraying methods. In this work, sodium alginate and gelatin were selected as the encapsulating material. A gelatin solution with the addition of sodium alginate was used as an aqueous solution of the gel-forming mixture. The best option is the composition of capsules containing 0.5% alginate and gelatin, the solution should be used at a temperature of 40Β° C. Capsules were obtained by the drip method. Capsules obtained from 0.5% sodium alginate were soft, with a smooth surface, with an average size of 2,0Γ—10-3ΠΌ

    The article presents the results of studies of the operating modes of the installation for encapsulation, as well as obtained data on the viscosity of the working fluid. At various frequencies of rotation of the peristaltic pump, the performance of this installation and the viscosity of the gelling fluid at various temperatures were determined. According to the results of the experiments, the optimal speed of the peristaltic pump for these nozzles (40 rpm) and the temperature of the gel-forming liquid (40 ΒΊΠ‘) were revealed

    Traditional Risk Factors of Acute Coronary Syndrome in Four Different Male Populations – Total Cholesterol Value Does Not Seem To Be Relevant Risk Factor

    Cardiovascular diseases are the most common cause of mortality and morbidity in most populations. As the traditional modifiable risk factors (smoking, hypertension, dyslipidemia, diabetes mellitus, and obesity) were defined decades ago, we decided to analyze recent data in patients who survived acute coronary syndrome (ACS). The Czech part of the study included data from 999 males, and compared them with the post-MONICA study(1,259 males, representing general population). The Lithuanianstudy included 479 male patients and 456 age-matched controls. The Kazakhstan part included 232 patients and 413 controls.In two countries, the most robust ACS risk factor was smoking (OR 3.85 in the Czech study and 5.76 in the Lithuanian study), followed by diabetes (OR 2.26 and 2.07) and hypertension (moderate risk elevation with OR 1.43 and 1.49). These factors did not influence the ACS risk in Kazakhstan. BMI had no significant effect on ACS and plasma cholesterol was surprisingly significantly lower (P<0.001) in patients than in controls in all countries (4.80Β±1.11 vs. 5.76Β±1.06 mmol/l in Czechs; 5.32Β±1.32 vs. 5.71Β±1.08 mmol/l in Lithuanians; 4.88Β±1.05 vs. 5.38Β±1.13 mmol/l in Kazakhs/Russians). Results from our study indicate substantial heterogeneity regarding major CVD risk factors in different populations with the exception of plasma total cholesterol which was inversely associated with ACS risk in all involved groups. These data reflect ethnical and geographical differences as well as changing pattern of cardiovascular risk profiles

    Zoning of the Territory of the Republic of Kazakhstan by the Degree of Intensity of the Epizootic Situation on Plague in Camels

    The aim of the study was to improve epizootiological monitoring and increase the effectiveness of preventive (anti-epidemic) measures for camel plague control in Kazakhstan.Materials and methods. We used the data on epizootiological and epidemiological monitoring in natural plague foci of Kazakhstan, long-term measurements and indicators for the period of 2000–2020 of the anti-plague and veterinary services of the Republic for the analysis. To process the evidence, epidemiological, epizootiological, microbiological, and statistical research methods, as well as GIS technology were applied.Results and discussion. The number of camels has increased by 2.2 times in Kazakhstan over the past 20 years.Β  Where there were 98.2 thousand heads in 2000, it amounted to 216.4 thousand heads in 2020. Over theΒ past 10 years, 152 camels died of unknown causes in the focal area of the country, but laboratory tests for plague turned out negative. According to the hazard criteria, the territory of the country has been conditionally divided into three zones: five regions with a high degree of hazard with a total area of 953.15 sq. km, five regions with medium degree of hazard with a total area of 1230.72 sq. km, and with a low degree of hazard – four regions and three cities of republican significance with a total area of 541.1 sq. km. Constant epizootiological monitoring over plague in camels is a necessity for the system of preventive measures

    THE EPIDEMIOLOGICAL SITUATION ON CHOLERA IN THE REPUBLIC OF KAZAKHSTAN IN 2011

    The analysis of registered cholera cases that occurred during 1993-2008 showed that all epidemic cases of cholera in Kazakhstan were imported. The toxigenic cholera microbes were isolated from the water reservoirs which started in the neighbor country. The spatial distribution of non-toxic cholera vibrio has the regional features. The climatic and anthropogenic factors affect the distribution of the cholera vibrio in the Kazakhstan. The results of the analysis show that the complex approach has to be used for cholera epidemiological monitoring
